Creating a Form

From the menu, select ‘Create New Form’.

You will then be asked for the following details :

  • Form Name – The name you would like to use to reference your form.
  • Thank You Page URL – The URL that you would like to send users to, once they complete the form. If you don’t have one, then just leave it blank and Monty will send them to the default thank you page.
  • Custom Fields – This where you decide which fields you would like to use in your form. Add as many or as few as you like.

Once you click ‘Create Form’, you form will be created.

*For your form to automatically populate the contact details in the deal view, you will need to use the following field names (including casing) :

  • Name
  • Phone Number
  • Email
  • Company
